To list items of more than average interest on standard 3 by 5 inch index cards, use the following form:

1. Composer or Author. (if little-known, indicate nationality). Upper Left corner of card.
2. Type of work (oboe method, bassoon solo, dissertation, etc.) Upper Right corner of card.
3. Name of work - describe its contents briefly. Below Composer or Author Name.
4. Publisher (if published) or Source (such as manuscript: "ms"). Include place and date of publication when possible; enclose in parentheses.
5. Further descriptive remarks of interest - follow the publisher or source information.
6. Location of the item. Private collection in this example, "gift to I.D.R.S." if you are donating an item.
